I wanted to personally let you know that, after much consideration, I have made the decision to leave Urology Clinics of North Texas.

I will be joining a Urology Group with UT Health in Athens, Texas. It has truly been an honor and a privilege to care for you and so many others throughout the years. I am incredibly grateful for the trust you have placed in me and for the opportunity to be a part of your healthcare journey.

My last day of practice at Urology Clinics of North Texas will be October 30, 2026.

If you have any questions about your care or need assistance in transitioning to another provider, please don’t hesitate to reach out. Our team is here to ensure a smooth transition and continued support.

Any future appointments with me will be rescheduled with a provider of your choice, either within or outside Urology Clinics of North Texas.

You will receive an email or text from Phreesia with a link to reschedule online, or you can contact us directly to arrange a new appointment at 214.826.6021.

Your medical records are stored in Urology Clinics of North Texas’ Electronic Medical Records (EMR) system and are accessible to other UCNT providers. If you choose to continue with another UCNT provider, no action is needed. Visit www.UrologyClinics.com for a list of providers and locations.

If you wish to transfer your records outside UCNT, you can request them via the “Patient Info” section on our website. Choose “Medical Records Release Authorization” to download it and print it. You may either fax it to the attention of the Medical Records Department at 214.823.9745; or mail it to UCNT Medical Records Dept. 3417 Gaston Ave., Ste. 830, Dallas, TX 75246
