This Website Privacy Policy (“Policy”) describes how Urology Clinics of North Texas, PLLC (“UCNT,” “we,” “us,” or “our”) collects, uses, and discloses information through our website located at urologyclinics.com (the “Site”).

It does not describe how we use and disclose your protected health information (PHI) as a patient. If you are contacting us through this Site to schedule an appointment, request patient portal access, or inquire about a clinical trial, please review the section below on Third-Party Services, as some of these requests are handled directly by outside platforms rather than by UCNT

A. Information You Provide Directly
The only information collected and stored directly by this Site is submitted through two forms, both of which collect the same limited set of fields:
- Homepage inquiry form: name, email address, and phone number
- Patient Portal access request form: name, email address, and phone number
We do not collect or store payment information, medical record numbers, Social Security numbers, or clinical health information through these website forms. Patient portal account login and access is managed entirely by Medfusion, a separate platform (see Section 3). Information submitted to schedule appointments or apply for clinical trial participation is handled by third-party platforms described in Section 3 below and is not stored on this website.
B. Information Collected Automatically
When you visit the Site, certain information may be collected automatically through cookies and similar tracking technologies, including:
- IP address
- Browser type and device information
- Pages visited and time spent on the Site
- Referring website or search terms
- General geographic location (derived from IP address)

Certain functions on this Site are provided by third-party platforms that operate independently of UCNT. When you use these features, you are subject to that third party’s own privacy policy and terms, in addition to this Policy.
- Phreesia — Used for online appointment scheduling. When you request or schedule an appointment through this Site, you are directed to or interact with Phreesia’s platform. UCNT does not store the scheduling request data submitted through Phreesia on this website.
- Realtime CTMS — Used to manage clinical trial applications. The clinical trial application form is embedded directly from the Realtime platform, a clinical trial management system. Submissions made through this form are transmitted to and managed by Realtime and are not stored on this website.
- Medfusion — Used as the Practice’s patient portal. Patient portal login and account access is provided entirely through Medfusion (medfusion.net), a separate platform. UCNT does not manage patient portal account credentials or store portal login data on this website. The form on this Site’s Patient Portal page is used only to request patient portal access information and collects your name, email address, and phone number so that our staff can respond to your request.
4. How We Use Information
We use the information described above to:
- Respond to inquiries submitted through website forms
- Understand how visitors use the Site so we can improve content and navigation
- Measure the performance of our online communications and, where applicable, advertising
- Maintain the security and proper functioning of the Site
5. Your Privacy Choices and Rights

When reasonably necessary, we may extend the response period by an additional 45 days as permitted by applicable law. If we extend the response period, we will notify you of the reason for the extension.
