I hope this message finds you well. We are writing to let you know that Dr. Muhammad Azharuddin will be leaving Cancer Clinics of North Texas at the end of June 2025. His last day seeing patients will be May 16, 2025.

Dr. Azharuddin is in the process of establishing a new practice with another local oncology group. Once this transition is confirmed, you will be notified and given the option to transfer your care to that location. You may also choose to continue your care with Dr. Anita Tharian at Cancer Clinics of North Texas.

If you already have an upcoming appointment scheduled at Cancer Clinics of North Texas, a member of our team will be in touch with you soon through phone, email, or text via our Phreesia online communication system. If you need to schedule a future appointment, please contact our office directly at 972-820-8774.

Your medical records will remain securely housed within Cancer Clinics of North Texas’ electronic medical record system, accessible to your future providers within our network. If you wish to transfer your care to a provider outside of our network, you can request your records by visiting www.cancerclinicsofnorthtexas.com. Simply click on the “Patient Info” link, select “Patient Forms,” and choose “Online Form” to submit a medical records request electronically. If you prefer, you can also download and complete the “Medical Records Release Authorization” form, then fax it to (office fax number) or mail it to CCNT (office location).
